Both act on 5-alpha reductase, finasteride potently and saw palmetto weakly. In the two-year head-to-head trial finasteride grew hair in 68 percent of men against 38 percent for saw palmetto, so the herb adds no documented benefit on top and muddies attribution if side effects appear.
